5 Doctors Answer: Should You Have Anal Sex?

By Laura Beck

. Since a lot of the information floating around out there about anal sex is purely anecdotal, Cosmopolitan.com spoke with five doctors about whether or not you should get into butt play. Here's what they told us.

Pro: It's Relatively Safe

1. "Medically there no reasons not to have anal sex, just follow safer sex practices as you're at a slightly higher risk for STI transmission. You also have to be mindful though of not having anal sex then vaginal sex without changing the condom." —Michael Krychman, MD

2. "From a medical standpoint, anal sex is safe if you take the usual safe sex precautions, such as condoms and dental dams, and loads of lube, since the anus is typically much tighter than the vagina. Many women enjoy anal play due to the incredibly rich nerve supply and heightened sensitivity and the added bonus of not having to worry about getting pregnant. On the flip side, many women are really turned off by anal sex and the fear of soiling themselves. Washing well and/or using an enema beforehand is an option that would make this less likely." —Alyssa Dweck, MD

3. "Anal sex is so pleasurable to many women because our bodies really do have a very, very rich cluster of nerves surrounding the anus. There's a reason why we like to be kissed on our lips, more than on maybe our elbow, because our lips have more nerve endings. That said, if you're engaging in anal sex, you have to take more precautions from an STD standpoint. If you're not in a mutually monogamous relationship then you should absolutely be using condoms for protection. Any time people have intercourse, even vaginal, there could be microscopic tears, which is how people catch viral STDs. But if you use lubricant, you'll reduce those tears, which could decrease your risk of transmitting a viral STD." —Jennifer Gunter, MD

Con: There's Increased Risk

4. "One concern about anal sex is related to the transmission of HPV. Certain strains of HPV do cause cancer, and with anal intercourse, HPV infections in this area can lead to anal cancer. There have been studies documenting that people who have had anal penetration by multiple partners are more likely to develop carcinoma of the anus, so be monogamous and use a condom. Other reasons to proceed with caution with anal sex is the risk of infection with E. Coli. You can also develop urinary tract infections, as well as infections of the bowel from anal intercourse. Repetitive tearing and injury to muscles of the anus can affect someone's ability to control their bowel movements, and can result in fecal incontinence or inability to hold stool effectively inside, until they make it to the bathroom. Although not common, a more serious injury such as a penetration or tear in the colon or bowel can occur, which would require an invasive surgery to correct." —Karen Elizabeth Boyle, MD, FACS

5. "Anal sex play cannot cause pregnancy, but it can easily pass infection. So if a condom breaks during anal sex, talk to a health care provider or your nearest Planned Parenthood health center about STD testing. If you're mainly having anal sex as a method of birth control, you can talk to the doctors while you're there about getting on birth control so you don't have to worry about pregnancy." —Vanessa Cullins, MD

Shocking News About Anal Sex

Whether you love it, hate it, have tried it or have thought about it, anal sex seems to be less taboo these days. But a new British study found that young women aren’t always doing it for the right reasons. The research, published in the medical journal BMJ Open, attempted to collect information about when anal sex occurs and the reasons why men and women engage in it.

Using a sample of 130 heterosexual men and women between the ages of 16 and 18 from the cities and suburbs of England, scientists conducted group interviews and individual interviews to find out about the participants’ sexual practices. The results revealed major gender differences in the ways men and women talk about anal sex and their motivations to have it.

While men in the study tended to correlate anal sex with pleasure and macho sexual achievement, women brought up a fear of physical pain and a damaged reputation. Male subjects often expressed that they wanted to have anal sex to mimic pornography and because it was more pleasurable than vaginal penetration, while women mainly said they did it to please their partners. There was a shared understanding between interviewees that women were supposed to be begged or forced into participating and should expect it to hurt, and if they turned down anal sex, they were uptight. The idea of “if you try it, you’ll like it” was frequently referred to as a tactic men used to convince women to try it. And unfortunately, many subjects were unaware that STIs can easily be transmitted through anal sex.

Obviously this study only looked at a small subsection of young adults in the U.K., so these attitudes are not representative of all men and women—especially older adults in committed relationships. Still, it’s shocking to see the damaging stereotypes and unfortunate gender norms surrounding anal sex.

However, these views aren’t necessarily uncommon among American adults as well, says licensed sex therapist Christine Milrod, Ph.D. She attributes this growing interest in anal sex—and the stereotypes surrounding the act—to pornography. But it’s important to remember thatporn is not sex education, and the themes you may see in it are often seriously unrealistic. The fact is that some women aren’t enthused about backdoor action, while others really enjoy it; some find it painful, while others find it pleasurable; and most importantly, it should never be something you’re forced into doing.

How to have anal sex

Anal sex is any type of sexual activity that involves the anal area and many people, whether they are heterosexual, gay or bisexual, enjoy it. Find out whether it’s right for you – and how to stay safe.

What is anal sex?

Most commonly, people think of anal sex as when a man’s penis enters the anus. However, it might also mean using fingers or sex toys to penetrate the anus, or using the tongue to stimulate the anus (called ‘rimming’).1 Everyone can enjoy anal sex, no matter whether they are gay, bisexual or straight.

How do I have anal sex?

It can feel strange when you start exploring the anal area during sex, so start slowly with touching and caressing to get used to the idea. If you don’t like it, it’s a good idea to talk to your partner and explain that anal sex isn’t for you. While lots of people enjoy it, many others would prefer to leave it out of their sexual activities. If you decide to have penetrative anal sex, take things slowly and communicate with your partner. If you are giving anal sex, start by penetrating just a little and then pull out completely. When your partner is ready, penetrate a bit further and then pull out again. Continue with this until you are fully in – but be prepared to stop at any time if the other person is uncomfortable or in pain. It can take a while to get used to the sensation of anal sex – if it doesn’t go perfectly the first time you can always try again when you’re both in the mood.

Love your lubrication

Unlike the vagina, the anus doesn’t produce its own lubrication (or ‘lube’), so it’s important to use a good product to help the penis or sex toy move freely and prevent damage to the inside of the anus.  Don't use your partner's come as a lubricant. It’s best to use a water-based lubricant which has been specially designed for sexual intercourse. Oil-based lubricants (such as lotion and moisturiser) can weaken condoms and make them more likely to break.1

Gay men and anal sex

Gay men can enjoy a range of different sexual activities, including oral sex, kissing and touching each other, as well as penetrative anal sex. Many men also like having their prostate stimulated. The prostate is a walnut-sized gland located just below the bladder and is highly sensitive to stimulation.2 However, there are many blood vessels in and around the prostate and it can get bruised if handled roughly, so always treat it gently and use lots of lube.3 "I thought that all that men did in bed together was anal sex, but no-one has ever tried to force me to have penetrative sex. It's much more of a loving, caring thing than I thought, too." - Peter

Safety for women having anal sex

For women, be careful not to use the same finger to stimulate her anus as you use to touch her vagina. This is because you could transfer small amounts of faeces to the vagina which can cause urinary tract infections such as cystitis. The same goes for using a finger to stimulate the anus and then putting it in the mouth, as this can pass on sexually transmitted infections (STIs) such as hepatitis. It’s also a good idea to put on a fresh condom to prevent these infections if you’re having vaginal sex or oral sex straight after anal sex.1  Technically, it’s not possible to get pregnant from anal sex as there’s no way for semen to get from the rectum into the vagina. But be aware that there is a small chance of semen leaking out and dripping into the vagina after anal sex.4 Using condoms is the best way to make sure you are always protected properly against pregnancy.

Anal sex, HIV and STI safety

Whether you’re a man or a woman, straight or gay, it’s important to protect yourself against the risk of sexually transmitted infections (STIs), including HIV, when having anal sex. The lining of the anus is thin and can easily be damaged, which makes it more vulnerable to infection. This means that you have a higher risk of STIs and HIV from anal sex than many other types of sexual activity.1

STIs that can be passed on during anal sex include:
HIV
Chlamydia
Genital herpes
Genital warts
Gonorrhoea
Syphilis.5

Using protection during anal sex is important to reduce your risk of catching an STI. For penetrative sex, make sure you use a condom and lots of lube – some people feel safer using extra-thick condoms for anal sex,6 and dental dams also offer good protection for rimming.  It’s a good idea to use condoms if you’re using sex toys for anal sex too, making sure you change them between partners and use a fresh one if you use the toys to stimulate the vagina afterwards.

Women Who’ve Tried Anal Sex Describe What It’s Actually Like

For some women out there, anal action is the cherry on top of the sex version of an ice cream sundae: a little extra delight that completes something that was already delicious on its own. But for others, it's more like pâté: intriguing enough, worth a try, but absolutely not up their alleys (as in, a penis will probably not be going up that alley ever again). We got women who fall on various parts of the anal sex spectrum to share exactly what it feels like during the act. Read on for their informative—and sometimes hilarious—opinions.

"If you're backed up or on an empty stomach, it sucks. You definitely feel like you're going to poop, either all over yourself or on his d*ck. But if you're not and you do it nice and slowly, it's euphoric. It's different from regular sex because it feels like he's going way deeper. Anal doesn't help me orgasm more easily, though." —Madeline R.

"I was always afraid it would hurt, but anal sex actually isn’t so much painful as it is uncomfortable. But! The discomfort is so extreme for some people that they can barely do it—like my best friend, who’s tried a few times with her fiancé and barely gotten it in, no matter how much lube they use. The key, apparently, is to be relaxed, which you really aren’t gonna be—in fact, knowing it’s about to happen will make you tense up more than usual—unless you happen to love it. I…do not love it, but my boyfriend is super into it, and he’s very respectful and lovely about not pressuring me. We maybe do it once every couple of months. I’m usually a little tipsy, which helps with the relaxation part and makes the whole thing a little easier. He’s a big advocate of using a buttplug beforehand to 'loosen everything up,' which may or may not work, but just draws it out more than I’d like." —Anna B.
